Waiver / Medical Release (All Counties) - All American Youth Activities of Florida, Inc registration form. Please keep in mind A.A.Y.A does not accept checks.

Self Defense Uniform Order Form (All Counties) - If you would like to purchase your child's uniform, please fill this form out completely and enclose cash only, with this form in an envelope. Uniforms take approximately 3-4 weeks to arrive after you've placed your order so please don't wait too long to order. All prices include sales tax.

Cheer America Uniform Order Form (All Counties) - If you would like to purchase your child's uniform, please fill this form out completely and enclose cash only, with this form in an envelope. Cheer Competition Time Schedules - Time Schedules for Saturday and Sunday

Dance Competition Registration Form (All Counties) - You must submit TWO copies of this form, along with payment, to your instructor.
